Prospective members have the ability to register within the secured online application portion of our Website. Information collected within this portion of the Website will be used for plan validation and enrollment purposes and may include the prospect member's name, address, phone number, DOB, Social Security number, Medicare ID number, Medicaid ID number, current insurance coverage, current health conditions and other pertinent information. Additionally, CarePlus may use supplemental information from third party sources such as a credit bureau or fraud protection service agency to verify the prospective member's identity.

By providing an e-mail address, you are indicating that publications can be sent to your e-mail address. You are not required at any time to submit your e-mail address for the purpose of receiving recurring informational/promotional e-mails from CarePlus Health Plans, Inc. and/or third parties. Health plan members may opt-out of receiving publications via e-mail by contacting the Member Services toll-free number that appears on your CarePlus member identification card or indicating your preference on our Website. CarePlus applicants may opt-out of receiving publications via e-mail by contacting the Member Services department at 1-800-794-5907.

When you log in to the secured section of the Website, you have the option to change your password, e-mail address, security question and response and communication preferences. In case you forget or misplace your password, we will use your security question and response to identify you before you can regain access to the site. To change your Personally Identifiable Information, call Member Services using the toll-free number on your CarePlus member identification card.

Technology Use on the CarePlus Website
